This form is designed to allow businesses, organizations and individuals to apply for reimbursement of necessary expenditures incurred due to the COVID-19 public health emergency. To receive reimbursement of your expenditures you must complete and submit the form, or at least the parts relevant to your qualified expenditures. The form is designed for universal use. Therefore, many sections of the form may not apply to you or your organization. For those non-applicable sections, check the “Not Applicable” box or type N/A.
Jasper County employees or their representatives may:
-request documentation to support all or any part of your request.
-visit and inspect physical premises and payroll and disbursement records for amounts claimed.
If the claimed reimbursements are deemed inaccurate based on review, the funds granted are subject to recoupment by Jasper County.
The CARES reimbursement application process has ended. The Jasper County Commission is no longer receiving applications for reimbursement.
